Chief Executive Officer

The Chief Executive Officer leads and manages the operations of Hospice. This includes ensuring excellence in corporate governance, strategic engagement and maintain organisational sustainability.

With a focus on ensuring that Hospice will continue to provide specialised palliative care, end of life choices, support and education so our community can experience death with dignity in a manner and place of their choice.

Clinical Nurse Manager

The Clinical Nurse Manager’s role is to lead Clinical activities and ensure that Hospice remains true to its values and continues to be a leader in the provision of specialised palliative care, end of life choices, support and education so our community can experience death with dignity in a manner and place of their choice.

Clinical Team

Clinical, Registered and Enrolled Nurses and Patient Care Assistants and the Support Coordinator provide all person-centred care 24hrs per day working under guidance of the GP, or medical practitioner and recommendations from the multi-disciplinary team.

Regular and ongoing palliative care education and training ensures that the clinical team deliver evidence based best practice care that is delivered in partnership with the patient and their family/carer.

Support Services Team

The Support Services Team includes a skilled and diverse team managing administration, finance, cleaning, catering, communications and engagement and volunteer management all working together to ensure the Hospice operates efficiently and effectively, supporting the clinical care team to provide the best care possible to patients and families.

Palliative Care Consultant

Our Palliative Care Consultant oversees the care of patients following referral from their Medical practitioner or GP. The Palliative Care Consultant attends weekly Multidisciplinary team meetings, which are held at Hospice to review the care and management of each admitted patient, looking at the person from a physical, emotional, psychospiritual and practical perspective, and making plans to address symptoms causing the most distress and impacting on that person’s quality of life.

The Multidisciplinary team consists of the Palliative Care Consultant, the MDT Medical Coordinator, The patients own GP or medical practitioner , Clinical Hospice Manager, Support Coordinator, Registered Nurses from Hospice, WA Country Health Regional Palliative Care team and Community team, Psychiatry registrar, Spiritual Care Coordinator and other specialities can be invited to join to bring a broader perspective to patient care, including representatives from Clinical Oncology and Neurological Nurses.

Support Coordinator

The Support Coordinator acts as a liaison and advocate providing your family/significant others with social, emotional, practical and psycho spiritual support needed and can link you to many community-based resources available to support you in your community.

Volunteers

Volunteers are a treasured and dedicated part of the care team. They donate their time, effort and love to provide support and companionship to our patients and families. They assist in all areas of operation including the board of management and supporting committees, clerical work, fundraising, reception, catering support, patient support and gardening.
